Analyse the pathophysiology of long-term health conditions and their impact on care planning. 3. Demonstrate cultural safety and responsiveness when supporting individuals and whanau with long-term care needs. 4. Critically evaluate models of hauora as applied to long-term care, with an emphasis on whanau-centred approaches.

- The portfolio is comprised of multiple components. To pass this module, students must meet all learning outcomes and gain a minimum mark of 50% in each component.
